Background:We aimed to evaluate the efficacy and safety of sildenafil (Viagra) in the treatment of ED patients who had no prior treatment options.
Methods:We conducted a systematic review of randomized controlled trials of sildenafil in ED patients with no previous treatment options and in men who had failed or refractory treatment. Patients were randomized to treatment or control groups with either sildenafil 25 mg, 100 mg, or 200 mg daily, or placebo (n=622). We reviewed abstracts, letters, and letters from patients who were included in the trial. We also reviewed the trial's primary outcomes, including response rates and tolerability.
Results:Sildenafil was not associated with any significant adverse events in any of the following subgroups: erectile dysfunction, cardiovascular disease, diabetes, and/or hepatic impairment.
Conclusions:This systematic review supports the use of sildenafil in patients who have no previous treatment options. Patients who had failed or refractory ED treatment were likely to have had an improved ED response, which may have improved sexual satisfaction and quality of life.
